How to Register with Brownlow Health

Anyone who would like to register with the Practice should ascertain if they are living within the Practice catchment area. You can do this by telephoning or visiting us and the reception staff will be happy to check that your address falls within the designated area.

If you wish to register with the Practice you should present at any of our 4 sites, Brownlow, Ropewalks, Student Health or SHAC (University of Liverpool Students only) where you will be given 2 forms to complete, a GMS1 (registration form) and a New Patient Information Form. Or you could print these forms off from this website and bring them with you.

Once the above forms have been completed the Reception staff will enter your details onto our medical database and make an appointment for a New Patient check if required

All new patients will be given a copy of our Practice Leaflet and will be offered the opportunity to be issued with a PIN number that will allow you to register with the Practice Online Services website. This will enable you to make and cancel appointments, update your address and telephone number if you move house and order repeat prescriptions on-line.

We endeavour to process registrations immediately to enable a smooth transition from one practice to another and to allow patients to access our services.

If you are a student at the University of Liverpool, we extend our Practice area map (as shown below) to cover any address within the following Liverpool Postcode areas: L1, L2, L3, L6, L7, L8, L15, L17 and L18.
